The St. Nicholas painting that you find in the Belfry of the Church appears to be loosely based off of "Saint Jerome Writing" by Caravaggio. The angle and placement of the skull including the open-page book underneath is nearly identical.
Might be a coincidence, but St. Jerome is known as a "Doctor of the Church" - though the usage of "Doctor" in the St. Nicholas painting is closer to a physician than a scholar.

Just tested in GIMP by squashing then adjusting the contrast/hue; it's a perfect match. Good find

I vaguely remember reading a developer interview where the artist (maybe Hiro Usuda?) was answering a question about the paintings and where their inspiration came from. The artist mentioned researching a lot of material but found one particular picture where something looked very creepy and out of place for a holy picture.
I couldn't find that interview, but I think might have been referring to this particular skull. Some Internet searching suggests that these types of skulls were used for Memento Mori. Rather fitting with the religious theme of Silent Hill 3 if true.
