Silent Hill 2 did this for the most part, only the Lying Figure had an introduction. It was actually quite effective. The mannequin just suddenly springs to life (a great surprise if you go right for the flashlight). Even if you were expecting nurses, playing the game on one of the harder modes gives you a nasty introduction to them, in which they are right on top of you the first time you encounter them and the initial camera angle makes it difficult to see them. The abstract daddy mooks are kind of a shock, as you probably aren't expecting boss mooks.
And, of course, Pyramid Head's first appearance comes with absolutely no warning. That all he does is stare at you, waiting in a place where you will be going in a minute, makes it all the worse.
^ This! Seeing him there just staring at you on the other side of the bars was scarier than actually having to fight him. It's incidents like that in the games that really get to you.
Same whenever you rounded down another hallway in the Woodside Apartments, and in the light shining through a far window you see a Mannequin standing there in "stand-by" mode. I thought the stand-by modes were scary too. It added a whole new level of... creepy.
